Ghost torpedoes? Refusing to fire torpedoes?

The least I want now is ghost torpedoes in this game. Or not sure what & how it actually happened, 1 or 2 times.

1895, France vs German fleets, all ships are my own designs, all balanced & fully playable, 1.02 it was.

My CL approached enemy CA, I switched torpedoes from off to "aggressive", at like 100m I did hear the torpedo launch sound & the indicator went on reload, I wanted to see how/where it goes, but I couldn't see anything because of smoke screen (and a slight lag it causes when there's too much going on in one place) around my ship & wake - nothing happened: no torpedo hit enemy CA, no sound of hitting. I thought maybe it missed & went further - looked around but saw nothing, it couldn't have missed though since both ships were almost parallel 100m apart.

 

Also similar situation - 1895, me in CA passing by on full speed enemy BB 200m away from me, torpedoes were switched to "aggressive" a bit earlier, angles are fine, all torpedoes on board, waiting, waiting... but it doesn't fire from a side underwater launcher & I see how my ship goes off the vantage position. I'm like WTF?! & quickly turn around the ship to at least use the stern tube. At 300m it did fire right away without any delay.  How/why?  I did fire torpedoes from a side at full speed even at 50m distance, but in this case it decided to not.

Actually there might be some minimal distance from the ship under which the torpedo sorta physically doesn't exist even if the launch sound was hear & indicator went on reload. Or it was intentionally set that under this distance it must not explode during hit.

Just had it again while testing my destroyer against TRs. There was like 5-10m between both ships when the torpedo was launched but again no hit or no visible sign of that torpedo going away.
