167% torpedo accuracy

I have noticed a few things I consider glitches.
Both my fleet and the enemy fleets have had, on occasion, over 100% accuracy with torpedoes, and are shown as having hit with more torpedoes that they carry.
When a ship is hit, and damage is shown on the main screen, it does not always show up on the target legend or "overall damage" score.  I've hit a DD with a 12 inch gun to see 1347 points come up in green, only to check the ship and see 147 points of damage done.

Most distressing:

I decide to engage in a battle knowing the odds are close.  When I look at my actual order of battle, I see my biggest battleships have on 75% structure, and/or that my escorting small ships have engine hits on them (and speed penalties) if given the option to end the battle because I can't locate my enemy, I am considered the looser because of damage assessed against me that existed prior to my even making a choice.
This last case should be easy to resolve.  Simply tell the player that there are ships in his command with damage from another source.
