Medals 10 and I am missing one each time?

I played 2 American campaigns and each time I only get 9 out of the 10 medals so I am wondering where along the campaign did I miss that 1 medal? If you are looking at your main officer page I have all 5 on the left side and only 4 on the right side the one I am missing is the one that would be far right top?

I will list when I got which medal around the time that I got it so maybe you can help me figure it out how to get the one I am missing?

1.  Take Her Back                    (Badge of Military Merit)

2.  The Battle of Yarmouth    (Navy Good Conduct Badge)

3.  Pain in the Neck                (Navy and Marine Corps Achievement Medal)

4.  Raid                                     (Navy Combat Action Medal)

5.  Battle of Valcour Island   (Navy and Marine Corps Medal Heroism)

6.  Another Attempt               (Defense Distinguished Service Medal)

7.  Battle of Cape Henry        (Navy Distinguished Service Medal)

8.  Unknown Enemy               (Navy Commendation Cross)

9.  Nemesis                             (Navy and Marine Corps Star)

As you can see from above I am missing out on #10 medal which I have no idea in two full campaigns where I went wrong not getting the medal wherever it maybe which is the one on top far right spot?

If any of you could help me with this one medal that would be great and I would be thankful?
